Endocrine tumors of the gastrointestinal tract and pancreas:grading, tumor size and proliferation index do not predict malignant behavior

چکیده
CONTEXT Gastrointestinal and pancreatic (GIP) endocrine tumors (ETs) have been regarded as slow growing neoplasms with distinct morphologic characteristics that behave less aggressively than carcinomas. The malignant potential of these tumors is difficult to predict. OBJECTIVE To evaluate prognostic parameters, namely tumor size, tumor grade, and Ki-67 index in relationship to metastatic behavior of GIP ETs. DESIGN Biopsies and surgical specimens from 38 patients with GIP ETs were selected. The study group comprised 16 males and 22 females (mean age 62.6 years; range 24-91). Formalin-fixed, paraffin-embedded tissue sections were stained with H&E, synaptophysin, chromogranin A, and Ki-67. Ki-67 index was evaluated using ChromaVision Automated Assisted Image Analysis software. Proliferative index was compared to tumor grade, and the degree of associations between tumor size, tumor grade, Ki-67 index and metastatic behavior of GIP ETs were evaluated. RESULTS Fifteen of the twenty-two (68.18%) surgically staged neoplasms presented with peritoneal dissemination, lymphogenous, and/or hematogenous metastases. Nine of the metastatic tumors were G1 (9/13, or 69.23%), 5 were G2 (5/7, or 71.42%), and 1 - G3 (1/2, or 50%). Overall, 10/15 (66.66%) metastatic tumors showed < 2% Ki-67 immunoreactivity. Four ileal ETs had a synchronous malignancy. No significant correlation was found to exist between tumor grade and Ki-67 index as well as between tumor size, tumor grade, Ki-67 index and metastatic behavior. CONCLUSION The findings suggest that tumor size, tumor grade and Ki-67 index do not accurately predict malignant behavior of GIP ETs.

Background & objective: Hemangiopericytoma is an uncommon vascular tumor derived from the pericytes of Zimmermann. . The most common location of the tumor is pelvic retroperitoneum and musculoskeletal system of the lower extremities. It is rarely seen in the head and neck area. In addition, only 0.8% to 3% of all orbital tumors and less than 5 % of primary sinonasal tumors are primary hemangiop...
متن کاملChemotherapy and role of the proliferation marker Ki-67 in digestive neuroendocrine tumors.
Neuroendocrine tumors (NETs) of the digestive tract are a heterogeneous group of rare malignancies. Three major subgroups can be defined: pancreatic endocrine tumors, carcinoid tumors, and poorly differentiated gastroenteropancreatic NETs. Classically, digestive NETS have been considered to have an indolent course characterized for prolonged stabilizations or slow progressions, but there are cl...
متن کاملPrediction of Clinical Course and Biologic Behavior of the Bone Giant Cell Tumor Using Bax and bcl-2 Markers
Background and Objectives: Giant cell Tumor of bone (GCT) is often regarded as a benign tumor, but its clinical course is unpredictable, has a high rate of recurrence, and even can metastases and transform to a malignant tumor. Histological features of the tumor often could not predict its future biologic behavior, so it is just called ''Giant cell tumor'' without indicating maligna...
متن کاملHistological and Immunological Evaluation of Gastrointestinal Stromal Tumors
Background: Gastrointestinal stromal tumors (GISTs) constitute the majority of gastrointestinal mesenchymal tumors. They usually express a proto-oncogen protein called CD117 detected by immunohistochemistry. This study investigated the differentiation of GISTs as well as the risk of aggressive behaviors in GISTs from surgically-treated patients in university affiliated hospitals. Methods: The c...

Neuroendocrine carcinoma (NEC) of stomach is a rare and highly malignant tumor. It rarely occurs in the gastrointestinal (GI) tract especially in stomach. In this report, an 81-year-old male is presented with chief complaint of epigastric pain and early satiety. Endoscopic examination of the upper GI tract revealed an ulcerative mass measuring 3x4x4 cm in body of stomach, and a biopsy was taken...
